Complete Your Enrollment Agreement

This page provides access to UMI's Enrollment Agreements. Select the Enrollment Agreement for the state in which you have been admitted. After completing the agreement, return to your Registration Form to continue the enrollment process.
STEP 1: SIGN ENROLLMENT AGREEMENT

If you have not yet completed your Enrollment Agreement, select the appropriate state below to review and electronically sign it.

After you receive the signed copy from UMI, download it and return to your Registration Form to upload it before continuing.

STEP 2: COMPLETE REGISTRATION FORM

After you receive the copy of your Enrollment Agreement signed by both you and a UMI representative, download it and return to your Registration Form to upload it before continuing with your enrollment.
