We look forward to seeing you join our program. Below, you will find the essential documents required to complete your enrollment process at United Medical Institute (UMI).
After completing your program application and successfully passing the Wonderlic Scholastic Level Exam (SLE), you may proceed with completing your Enrollment Agreement documents. This step outlines the terms and conditions of your enrollment. Please note that your place in the program will not be secured until you also complete the Registration Form, finalize your tuition financing/payment options, and pay all applicable enrollment fees.
After completing your Enrollment Agreement documents, you may proceed with the Program Registration form. In this form, you will submit additional registration documents to create your student record, make the final selection of your tuition payment/financing options, and secure your spot in the program.
